Rotten Breath is not a field loot pickup. You buy it from the Dragon Communion altar at the Cathedral of Dragon Communion in Caelid for one Dragon Heart. The same spell is also available later at the Grand Altar of Dragon Communion on the Jagged Peak.
Reach southern Caelid and head to the Cathedral of Dragon Communion — the ruined church southwest of the Caelid rot flats, not the early Church of Dragon Communion on the coastal island in Limgrave. Rest at the Cathedral of Dragon Communion Site of Grace if you have it, approach the altar inside, and exchange one Dragon Heart. You need a Dragon Heart first (defeat any dragon that drops one). After the purchase, memorize Rotten Breath at a Site of Grace and equip a Sacred Seal that meets Faith 15 and Arcane 12.

Rotten Breath Damage & Scarlet Rot Effect
Direct breath damage plus Scarlet Rot buildup — the status is the main reason to cast it.
Rotten Breath channels a dragon and spews a continuous Scarlet Rot breath. Each connected tick deals Standard (physical) damage from Incantation Scaling and adds Scarlet Rot buildup; holding the cast is how you finish the status.

A full cast can fire two breath waves. Keep the stream on the target when it is safe — missed hits waste both damage and Scarlet Rot buildup.
Direct damage scales with your seal's Incantation Scaling. Scarlet Rot buildup is fixed per hit and does not go up with Arcane.

Short taps only chip the Scarlet Rot bar. Hold for full waves when you need the status to actually land on tougher enemies.
Large, slow targets and open ground favor the breath. Tight rooms, highly mobile enemies, and anything that knocks you out of the cast make Scarlet Rot harder to finish.
If the rot bar barely moves, the target is resisting or immune — stop spending FP and switch tools. Ekzykes's Decay builds the same amount per hit, so it will not fix an immune fight either.
Best Seal and Scaling for Rotten Breath
Any Sacred Seal works once you meet Faith 15 and Arcane 12.
Dragon Communion Seal is the natural pairing: Faith + Arcane scaling, plus about +15% direct damage per seal in an active hand (two max).
Roar Medallion (~+10%) and Rock Heart (~+20%) also raise direct damage only. They do not increase Scarlet Rot buildup per hit.

How to Apply Scarlet Rot
Hold for full breath waves instead of panic taps — that is how Scarlet Rot actually fills.
Cast when the enemy is locked in a long animation, recovering, or stuck on a wide path you can track.
After Scarlet Rot lands, stop holding and play safer while the damage-over-time runs. Reapply only if the fight outlasts the status.
A full two-breath cast costs about 120 FP in total. The second wave can finish once it has started even if FP runs dry mid-stream, but you still need enough FP to begin that second wave.
Get your footing first. Getting hit out of the cast wastes FP and resets your chance to apply rot.
Rotten Breath vs Ekzykes's Decay
Cheaper ground breath now, or stronger airborne Scarlet Rot later — same rot per hit, different damage and cast.
Both are Dragon Communion Scarlet Rot breaths from the Caelid altar. They apply the same Scarlet Rot per hit. Ekzykes's Decay hits harder on each breath, costs more FP, and casts from above — including while jumping.

Scarlet Rot per hit is the same. What you gain with Ekzykes's Decay is stronger direct damage, a better cast angle, and jump-casting — not a higher rot number.
Most loadouts only need one of the two. Grab Rotten Breath first; move to Ekzykes's Decay later if you want the stronger breath.

You are still around Faith 15 / Arcane 12, only have one Dragon Heart, or need Scarlet Rot breath before Decaying Ekzykes is down.

Is Rotten Breath Good?
Rotten Breath is one of the best early Scarlet Rot options in the Dragon Communion set: low requirements, one Dragon Heart, and strong damage over time once the status sticks on a target that is not immune. Use it alongside real weapon or spell damage — not as your only plan. Switch to Ekzykes's Decay when you can unlock it and want more direct damage for the same Scarlet Rot per hit.
PvE
Best in open fields where you can hold the stream — field dragons, large wildlife, and wide Caelid approaches.
A common use is Mother Greyoll in northeast Caelid: stay on her flank, apply Scarlet Rot, and let the damage over time work.
Weak when the target is highly mobile, keeps interrupting the cast, or the rot bar does not move.
Bosses
Use on bosses that can take Scarlet Rot and give you a long opening in open space.
Skip fights where rot does nothing — Malenia does not take Scarlet Rot, so the breath is wasted FP there.
If the arena is a tight corridor and every cast gets interrupted, bring a faster spell or weapon instead.
PvP
Long startup and a readable breath cone make it easy to punish in duels.
Keep it for PvE Scarlet Rot value; it is not a primary PvP plan.
